Case Manager

About the role

AMG Specialty Hospital – Central Indiana (Muncie Campus) is seeking a full‑time Case Manager (RN or LPN) to provide special services that facilitate patients’ continuity of care after discharge. The role involves coordinating admission and discharge planning, case communications, insurance relations, and ensuring compliance with patient‑rights regulations. The position is located on the 5th floor of IU Health Ball Memorial Hospital in Muncie, IN.

Responsibilities

• Make special services readily available to patients, families, and significant others to support post‑discharge adjustment.
• Oversee effective coordination of admission and discharge planning.
• Manage case communications, conferencing, and insurance relations.
• Ensure assurance through managed‑care systems and maintain verbal and written reporting.
• Assure that regulations regarding patient rights are fulfilled.

Requirements

• Current Indiana Registered Nurse (RN) license or Licensed Practical Nurse (LPN) license in good standing.
• Current Basic Life Support (BLS) certification.
• Current Advanced Life Support (ALS) certification.
• Minimum of 1–3 years of related work experience as a Case Manager.
• Ability to read and communicate effectively in English.
