Through advocacy, resource coordination, and evidence-based practices, you'll help clients navigate challenges, access services, and make meaningful progress on their recovery journey while fostering independence and self-sufficiency.

Responsibilities

  • Provides psychosocial rehabilitative services as described by the Medicaid manual
  • Serves as a liaison between clients and various community resources
  • Supports clients in following individualized care plans and identifying case management goals
  • Completes documentation on services provided within Valley’s standards
  • Maintains accurate records and reports as required
  • Maintains confidentiality of client information
  • Distributes medications as necessary
  • Transports clients to appointments as necessary

Requirements

  • High School diploma or equivalent
  • CPR certification
  • Valley de-escalation certification
  • Minimum age of 21 for driving positions
  • Valid driver's license for driving positions

Pay

Dependent on Case Manager certification status Uncertified starts at $17.50 - certification will be obtained within the first 120 days and pay will be increased to certified rateCertified range starts at $19.61 - pay is calculated based on years of certified experience

Schedule

Tuesday - Saturday | 8:00am – 4:00pm
