About the role

Support Growth, Recovery, And Success At Adult Outpatient. At Valley West, you'll help clients build the skills, confidence, and resources they need to thrive. As a Case Manager, you'll provide compassionate, goal-focused support while partnering with clients to promote recovery, independence, and long-term success.

Responsibilities

  • Provides psychosocial rehabilitative services as described by the Medicaid manual
  • Supports clients in following individualized care plans and identifying case management goals
  • Completes documentation on services provided within Valley’s standards
  • Meets productivity expectations as assigned by supervisor
  • Distributes medications as necessary
  • Transports clients to appointments as necessary

Requirements

  • Case Manager certification (post hire as needed - required within 120 days of start date)
  • CPR certification (post hire as needed)
  • Driving positions require a minimum age of 21 and a current driver’s license

Qualifications

  • Previous social services or behavioral health experience (preferred)
  • Bachelor’s degree in Social Services, Psychology, or related field (preferred)

Pay

Dependent on Case Manager certification status:

  • Uncertified starts at $17.50 - certification will be obtained within the first 120 days and pay will be increased to certified rate
  • Certified range starts at $18.75 - pay is calculated based on years of certified experience

Schedule

Tuesday – Friday | 9:30am – 8:00pm
