Case Manager - Adult Mental Health

Make a meaningful difference in the lives of adults living with a mental health illness. Hampton-Newport News Community Services Board (H-NNCSB) is hiring a full-time Adult Mental Health Case Manager to provide behavioral health case management, care coordination, advocacy, and supportive services to adults in the Hampton and Newport News communities. Since 1971, H-NNCSB has provided behavioral health services to residents of Hampton and Newport News, supporting individuals affected by mental illness, developmental disabilities, and substance use disorders.

Responsibilities

• Assess client needs
• Coordinate services
• Develop and monitor treatment plans
• Connect individuals with community resources
• Provide supportive counseling and advocacy
• Respond appropriately to urgent or crisis situations

Requirements

• Bachelor's degree in a Human Services field (such as Social Work, Psychology, Sociology, Counseling)
• One (1) year of case management experience

$50,604 annual salary
