CASE MANAGER, AACS HEALTHY TRANSITIONS- CHARLOTTESVILLE (FT)

About the Role

This is a professional-level, FLSA non-exempt position. The Case Manager provides Care Coordination services to individuals aged 18 or older with a primary diagnosis of mental health and/or substance abuse concerns, who are involved in the Criminal Justice System or recently released from a correctional facility. Responsibilities include developing service access plans, liaising with consumers, families, programs, and service providers, and ongoing monitoring of consumer needs. The role involves advocacy, consultation, and education for consumers, families, and the community. Time is split between the office at Region Ten and the Albemarle Charlottesville Regional Jail (ACRJ).

Responsibilities

• Enhance community integration through increased opportunities for community access and involvement, promoting community living skills and adjustment.
• Make collateral contacts with individuals' significant others, with authorized releases, to promote implementation of individualized service plans.
• Complete release planning at ACRJ for consumers requiring mental health or substance use disorder services upon release, as requested by ACRJ staff or local collateral contacts.
• Maintain regular meetings with branches of the Criminal Justice Community (Local, State, Federal Probation) to assist with consumer coordination of care and ensure required releases are present.
• Link individuals to community supports that promote personal habilitative/rehabilitative and life goals.
• Assure coordination of services and service planning within provider agencies, with other providers, and with other human service agencies.
• Monitor service delivery through contacts with individuals, service providers, and periodic site/home visits to assess care quality and satisfaction.
• Provide Case Management for all Healthy Transitions consumers, including linkage with other services and collection of required program data.
• Advocate for individuals in response to their changing needs.
• Plan for transitions and re-entry needs for individuals within ACRJ and other correctional settings.
• Monitor individuals' health status, medical conditions, medications, and potential side effects; assist in accessing primary care.
• Maintain complete and current consumer records, reports, and documentation in compliance with agency, state, and federal standards.
• Maintain Healthy Transitions and related Criminal Justice data to support funding and track outcomes.
• Read all agency communications (e.g., newsletters, emails).
• Perform other duties as assigned by the supervisor.

Requirements

• Bachelor’s degree in Social Work, Psychology, Sociology, and/or Human Services field.
• Minimum of 1 year of work experience, preferably with individuals with substance-use disorder or mental health disorders.
• QMHP certification is required.
• Valid Virginia Driver’s License with an acceptable driving record; valid personal automobile insurance for business use of a personal car.

Skills & Knowledge

Knowledge of: Populations with intellectual disability, mental health disorders, and substance use disorders; DSM 5; least restrictive treatment alternatives; medical necessity; general principles of record keeping; levels of care.

Skills in: Case management; linkage to individual and group counseling; interviewing; crisis management; DSM diagnosis; engaging resistant or difficult-to-engage clients; working with individuals involved in the criminal justice system; computer proficiency.

Abilities to: Establish rapport with clients and their families; maintain effective working relationships with a diverse group of professionals; accept and seek supervision when needed; communicate effectively with clients and other professionals; work independently.
