Essential Duties
- Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills.
- Provide direct care for ADLs and group activities.
- Lift and transfer individuals as appropriate to their needs.
- Cookordinate services with Interdisciplinary Team.
- Communicate respectfully and pro-actively with families, residential staff and all members of participants’ teams.
- Develop and implement Day Habilitation Service Plans (DHSP), including:
- Maintain progress notes and document progress on objectives.
- Maintain assigned participant records so that all information is current and filed appropriately.
- Write bi-annual and annual summaries.
- Complete all required documentation in a timely and accurate manner.
- Participate in scheduled supervision sessions with Assistant Director.
- Participate in program-wide staff meetings and case review meetings as scheduled.
- Participate in team meetings regarding general program development, including curriculum development and Schedules.
- Ability to demonstrate good computer skills as well ability to learn to utilize the agency’s computer programs for documentation of necessary data.
- Attend in-service trainings as required.
- Ability to work in a team environment.
- Ability to multitask and prioritize tasks.
- Strong written and verbal communication skills along with excellent interpersonal skills.
Requirements
- Experience working with Individuals with ID/DD
- A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
- Valid Driver's License required
- Valid MAP certification is preferred
- Excellent verbal and written communication skills
