Essential Duties
- Assists individuals served within the program obtain and maintain employment that is consistent with their employment goals.
- Collaborating with an exceptional team to plan and engage in the advocacy and support of our individuals, attending regular meetings as necessary.
- Serving as a support for individual’s self-employment, education, social growth, health wellness, communication skills, community mobility, and volunteerism.
- Providing support through case management (electronic (Icentrix) and physical case records).
- Writing and implementing Individual Support Plans (ISP) and completing all documentation.
- Making monthly and weekly progress notes for individual goals and objectives.
- Maintaining consistent communication with individuals and members of the ISP team.
- Providing on and off-site support for Life Enhancement Curriculum:
- Aiding individuals served in areas of employment, social, creative, and health & wellness development and training.
- Leading individuals served in community volunteer and learning experiences.
- Daily completion of payroll and billing documents.
Requirements
- Experience working with Individuals with ID/DD
- A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks.
- Valid Driver's License required.
- Valid MAP certification is preferred.
- Excellent verbal and written communication skills.
