Case Management Coordinator - Field (Mason, Cass, Brown, Scott and surrounding counties)

About the role

The Case Management Coordinator at CVS Health is responsible for collaborating and informing the case management process. They assist with appointment scheduling, identify and assist members with accessing benefits and education through the use of care management tools and resources.

Responsibilities

• Evaluation of Members: Conduct comprehensive evaluations of member’s needs/eligibility and recommend approaches to case resolution and care planning.
• Enhancement of Medical Appropriateness and Quality of Care: Consult with case managers, supervisors, Medical Directors, and other health programs to improve care plans and address quality of care issues.
• Monitoring, Evaluation and Documentation of Care: Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines.

Requirements

• 2+ years experience in Behavioral Health, Social Services, or a related field.
• Willingness and ability to travel 50-75% of the time in Mason, Cass, Scott and Brown counties, Illinois.
• Reliable transportation required.
• Experience with electronic documentation and Microsoft Office applications.

Qualifications

• Bachelor's degree or non-licensed master level clinician required in Behavioral Health or Human Services (Psychology, Social Work, Marriage and Family therapy, Counseling).

Preferred Qualifications

• Case management and discharge planning experience.
• Managed Care experience.
