Primary Responsibilities
- Support the Resident and Family Supportive Programs by providing case management functions and other non-clinical services to residents.
- Be available to respond within the scope of the program, on a pre-scheduled on-call basis for scheduled and/or emergency/unscheduled escorts.
- Escort residents to healthcare provider offices, emergency departments or other healthcare settings.
- Provide support for virtual/phone healthcare provider appointments.
- Accompany residents to other non-medical and non-financial appointments such as hair appointments, shopping trips, etc.
- Fulfill the requirements of an escort, including arranging/confirming transportation, communicating expressed/documented information, and coordinating support.
Compensation
- $21.30 / hour for scheduled work performed (some travel time may be included).
- $31.30 / hour for unscheduled, emergency response during a scheduled shift (travel time included).
- $2 / hour bonus pay for time scheduled on call but not worked.
- Mileage reimbursement at current federal rate.
About the Role
Era Living is seeking on-call Escort Case Management Assistants to join their Resident and Family Supportive Programs team in Seattle, WA. Preference is given to those available for the overnight shift from 8:00PM to 8:00AM.

Knowledge, Skills, and Abilities
- Work effectively as part of a multidisciplinary team.
- Multi-task and attend to detail.
- Facilitate communication and interaction with residents, family members, and/or other third parties.
- Travel to multiple Era Living communities to perform duties as required.
- Respond and travel urgently during on-call shift to meet resident at healthcare provider location.
- Use computers, Word, Excel, Outlook, and the Internet including smartphone applications.
- Foster a positive environment and represent Era Living in a proficient and professional manner; maintain professional boundaries and use discretion in consideration of stated visits.
- Available to work at least 4 shifts per month.
- Read, write, and communicate in English.
- Pass DSHS background check, required.
Education and Experience
- High School or GED, required.
- Bachelor’s Degree in human services, social work, gerontology etc., preferred.
- Experience working with seniors, preferred.
- First Aid and CPR certified or able to obtain within 30 days of hire.
- Complete Caregiver Dementia and Caregiver Mental Health training within 120 days of hire.
