Job Summary and Qualifications

The Case Management Assistant (CMA) at HCA Florida Northwest Hospital works under the direction of the RN Case Manager or Social Worker to facilitate patient discharges requiring post-acute placement or services, and to identify and report barriers to discharges. The CMA coordinates timely referrals and facilitates payor authorizations where applicable. Responsibilities include supporting multidisciplinary rounds, making referrals, monitoring PAC referrals, and communicating with case managers and social workers.

Responsibilities

• Coordinates with RN Case Manager and Social Worker for support needed.
• Communicates updates on progress throughout the day.
• Refers patient questions back to the case manager/social worker.
• Provides clerical support of multidisciplinary rounds, including updating NATE Tempo.
• Makes referrals through the web-based referral system (nH Discharge/Other).
• Makes appointments for outpatient hemodialysis.
• Makes referrals to PAC providers and sends necessary information.
• Communicates with RN Case Managers/Social Workers regarding the status of PAC referrals, bed availability, and other barriers.
• Sends additional information to PAC providers when requested.
• Communicates results of PAC referrals with the patient, including having the Patient Choice letter signed.
