CASAC-T - Transitional Housing

About the role

We're looking for a compassionate and dedicated CASAC-T to join our 24-Hour Transitional Housing Program. In this role, you'll provide substance use counseling, recovery support, and care coordination to adults transitioning from homelessness, hospitalization, treatment programs, or other levels of care.

Responsibilities

• Provide individual substance use counseling using a person-centered, recovery-oriented approach.
• Facilitate psychoeducational, relapse prevention, and recovery support groups.
• Complete substance use screenings, assessments, and individualized treatment plans.
• Conduct 90-day treatment plan reviews and monitor residents' progress toward recovery goals.
• Manage a caseload of residents with current or past substance use disorders.
• Collaborate with Case Managers, Social Workers, Clinical Supervisors, nursing staff, and community providers to coordinate comprehensive care.
• Assist residents with discharge planning by connecting them to outpatient treatment, detoxification, rehabilitation, medication-assisted treatment (MAT), mental health services, and community resources.
• Advocate for residents to ensure timely access to behavioral health, medical, housing, and supportive services.
• Respond appropriately to behavioral health and substance use crises using trauma-informed and de-escalation techniques.
• Maintain accurate, timely, and compliant documentation in the Foothold/AWARDS electronic health record.
• Participate in multidisciplinary team meetings, case conferences, and required trainings.
• Engage family members and natural supports, when appropriate, to strengthen each resident's recovery plan.
• Promote life skills, relapse prevention strategies, harm reduction, and healthy coping mechanisms that support long-term independence.

Requirements

The following qualifications are required:

• 1 year of experience in a residential setting preferred
• CASAC-T credential in good standing with the New York State Office of Addiction Services and Supports (OASAS)
• Strong interpersonal, counseling, and communication skills
• Excellent organizational, documentation, and time management abilities
• Ability to work both independently and collaboratively within a multidisciplinary team

Preferred Qualifications

The following qualifications are preferred:

• Experience working in transitional housing, residential treatment, supportive housing, behavioral health, homeless services, detoxification, rehabilitation, or community-based programs
• Experience working with adults experiencing co-occurring mental health and substance use disorders
• Knowledge of Motivational Interviewing, Harm Reduction, Trauma-Informed Care, Recovery-Oriented Care, and Crisis Intervention
• Experience using electronic health records, preferably Foothold/AWARDS
• Bilingual English/Spanish is a plus

Pay

The compensation for this role is $50,000 per year.

This is a full-time position, working Tuesday-Saturday with 8-hour shifts and start times between 09:00am, 09:30am, 10:00am, or 11:00am.
