Caregiver Full Time 3pm-11pm

About the role

As a Caregiver at Richwood Nursing & Rehab – The Oaks in LaGrange, Kentucky, your primary purpose is to provide direct care to residents of the assisted living facility. You will also perform general housekeeping/cleaning to maintain a sanitary environment and complete tasks as directed by supervisors.

Responsibilities

• Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
• Perform general housekeeping duties.
• Report changes in residents’ condition to the Health and Wellness Director promptly.
• Record entries on notes, SPAs, incident reports, etc., in an informative, descriptive manner.
• Report all accidents and incidents observed during the shift to the Executive Director and Health and Wellness Director.
• Agree not to disclose residents’ protected health information and report suspected or known violations.
• Report any known or suspected unauthorized attempts to access the community’s information system.
• Perform only care tasks for which you have been trained.
• Ensure residents’ rooms are ready (e.g., bed made), greet residents, and escort them to their rooms.
• Introduce residents to roommates and other residents or personnel as appropriate.
• Make residents comfortable and inventory/mark personal possessions as instructed.
• Store residents’ clothing and assist with packing personal possessions during transfers or discharge.
• Transport residents to new rooms or receiving areas and assist with loading/unloading from vehicles.
• Create and maintain a warm, positive, and calm environment throughout the unit and shift.
• Meet regularly with shift nursing personnel to identify and correct problem areas or improve services.
• Report all resident complaints and grievances.
• Participate in and receive the nursing report as instructed.
• Follow policies concerning exposure to blood/body fluids.
• Make beds and add extra covers as requested.
• Ensure residents who cannot call for help are checked frequently; answer resident calls promptly.
• Check residents routinely to ensure personal care needs are met.
• Assist residents with identifying food arrangements (e.g., describing food location, temperature).
• Keep residents’ water pitchers clean, filled with fresh water each shift, and within easy reach.
• Perform after-meal care (remove trays, clean residents’ hands, face, clothing, etc.).
• Receive the nursing report upon reporting for duty.
• Perform all assigned tasks in accordance with established policies, procedures, and supervisor instructions.
• Follow work assignments and schedules to complete tasks.
• Cooperate with inter-departmental and other facility personnel to maintain services for residents.
• Notify the facility when late or absent from work.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per facility policies.
• Assist residents with daily functions: dental/mouth care, bathing, hair care, dressing/undressing.
• Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
• Assist residents in preparing for medical tests (lab work, x-ray, therapy, dental, etc.).
• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Perform restorative and rehabilitative procedures as instructed.
• Assist in preparing residents for physical examinations.
• Check residents routinely to ensure personal care needs are met per their wishes.
• Observe and report pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores).
• Report injuries of unknown source, including skin tears.
• Observe disoriented and comatose residents; record and report data as instructed.
• Perform special treatments as instructed.
• Immediately notify the Nurse of any resident leaving/missing from the facility.
• Use only equipment for which you have been trained; report defective equipment to the Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of all resident care information, including protected health information.
• Report known or suspected unauthorized disclosure of resident information.
• Report all allegations of resident abuse and/or misappropriation of resident property.
• Honor residents’ refusal of treatment requests and report such requests to supervisors.
• Give back rubs as instructed.
• Assist with dressing/undressing, hair care (combing, brushing, shampooing), and nail care (clipping, trimming, cleaning).
• Shave male residents and keep hair on female residents clean as instructed.
• Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
• Make beds (occupied and unoccupied) and add extra covers as requested.
• Position bedrest residents correctly and comfortably.
• Assist residents with bowel and bladder functions (bathroom, bedpan, urinal, portable commode).
• Maintain intake and output records as instructed.
• Keep incontinent residents clean and dry; check and report bowel movements and stool character.
• Utilize protective and adaptive equipment as instructed.
• Provide eye and ear care (e.g., hearing aid installation, cleaning eyeglasses/hearing aids).
• Weigh and measure residents as instructed; measure and record temperatures, pulse, and respirations (TPRs).
• Ensure residents who cannot call for help are checked frequently.
• Assist with the care of end-of-life residents and perform post-mortem care as instructed.

Qualifications

• Must possess, as a minimum, a high school education or its equivalent.

Skills

• Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations; write reports, business correspondence, and procedure manuals; present information and respond to questions from managers and employees.
• Mathematical Skills: Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
• Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations with limited standardization; interpret instructions in written, oral, diagram, or schedule form.
• Technical Proficiency: Proficient in Microsoft Suite products.

Physical Demands

• The employee must occasionally lift and/or move up to 25 pounds.
• Prolonged use of desktop or laptop computers.
• Regularly required to sit, stand, walk, and talk; read or hear.
• Frequent use of office-related equipment including copier/scanner/fax, telephone, and calculator.
