Care Transitions Coordinator Home Health,
Responsibilities
Affords patients the opportunity to navigate post-acute care in a way that positively impacts patient outcomes and referral source satisfaction.
Utilizes evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the creation of transition plans that are patient-centric, aiming for quality and efficiency in post-acute care delivery.
Represents the branch in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, physician groups, and executive-level opportunities.
Qualifications
- Graduate of an approved school of nursing, therapy, or social work
- Licensed in the state where practicing
- Two years' demonstrated field experience
- Demonstrated understanding of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice operations
- Basic demonstrated technology skills, including operation of a mobile device
Requirements
- Valid state driver license
- Automobile liability insurance as required by law
- Dependable transportation in good working condition
- Safety in operating an automobile in all weather conditions
