Care Transitions Coordinator Home Health,
Responsibilities
Affords patients the opportunity to navigate post-acute care processes, aiming to enhance patient outcomes and improve referral source satisfaction.
Utilizes evidence-based clinical guidelines, preventative strategies, protocols, and metrics to develop transition plans that are tailored to individual patients, ensuring high-quality and efficient post-acute care delivery.
Engages in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, and physician groups, as well as with executive-level stakeholders, to advance the organization's goals and initiatives.
Requirements
- Must be a graduate of an approved nursing, therapy, or social work program.
- Must be licensed in the state where they practice.
- Must have at least two years of relevant field experience.
- Must demonstrate knowledge of federal, state, and local regulations governing Medicare-certified home health and hospice operations.
- Must possess basic technology skills, including the ability to operate a mobile device.
Qualifications
- Education and Experience, Essential:
- Graduate from an approved nursing, therapy, or social work program.
- Licensed to practice in the state where they work.
- At least two years of field experience.
- Comprehensive understanding of federal, state, and local laws and regulatory guidelines applicable to home health and hospice operations.
- Basic technological proficiency, including the ability to use a mobile device.
Preferred Qualifications
- Registered Nurse or Physical Therapist preferred.
- Three years of field experience preferred.
- Previous experience in home health or healthcare sales preferred.
