Care Team Manager
About the role
As an integral component of the leadership team at Tidewell Hospice, the Care Team Manager conveys the organization’s Mission/Vision to the community served. This role ensures compliance with governing state and federal regulations and accreditation standards. Through oversight and supervision of clinical and non-clinical staff, the Care Team Manager maintains quality patient care in alignment with organizational objectives and policies by planning, coordinating, implementing, and evaluating services provided to patients and families.
Responsibilities
- Promote and model the organization’s vision, mission, and values, aligning decisions and strategies with business goals, industry trends, and external forces.
- Develop and execute goals and strategic plans for your area of responsibility and the broader organization to achieve exceptional results.
- Lead and manage a diverse clinical and non-clinical staff (e.g., nurses, home health aides), including attracting, developing, and retaining a high-performing team. Assess performance, provide coaching/counseling, and complete formal performance evaluations.
- Oversee staff and make sound decisions with patient/client care in mind, including developing and implementing best practices to enhance the patient/client experience.
- Seek and respond to feedback from internal/external customers to improve service and patient care, managing service recovery efforts thoroughly and timely.
- Develop and execute an audit process to ensure regulatory and accreditation requirements are met.
- Analyze financial metrics and implement strategies to guide business decisions and meet or exceed goals.
- Communicate in a timely, clear, and concise manner with internal and external leaders, team members, and community partners.
Requirements
- Bachelor’s degree (B.A.) from a four-year college or university; Master’s degree preferred; or equivalent combination of education and experience.
- Two years of hospice experience with at least two years of supervisory management experience preferred.
- Seven to ten total years of experience as an RN or LCSW preferred.
- Licensure: RN or LCSW and/or equivalent experience.
- Must acquire continuing education and training related to regulatory and professional practices, demonstrating advancement in knowledge and expertise.
- Valid CPR certification from the American Red Cross or American Heart Association.
- Current certifications as applicable to professional discipline.
- Reliable transportation and valid automobile insurance (proof required).
Physical Requirements
- Push, pull, or lift 50 lbs. (e.g., positioning patients, moving equipment, transferring patients).
- Support 50 lbs. of weight (e.g., ambulating patients, assisting to bed or toilet).
- Carry equipment and supplies.
- Use upper body strength (e.g., performing CPR, physically restraining patients).
