Care Team Manager
Beacon is a national private-equity backed behavioral health company providing residential-based services and supports to children and adults with Intellectual and Developmental Disabilities, Severe Mental Illness, Autism, and high-acuity behaviors. All activities related to this position must be delivered in compliance with the company’s policies and procedures, accreditation standards, state and federal licensing requirements, HCBS guidelines, and the contractual requirements of Community Mental Health Agencies and other regulatory bodies. Responsibilities
- Operate assigned home(s) in compliance with all company and regulatory policies and procedures to meet or exceed home budgets and clinical effectiveness goals.
- Support and supervise assigned Direct Support Professionals (DSPs), ensuring they have the training and skills to provide high-quality care.
- Maintain assigned home(s) and ensure good working order of all related systems, facilities, and furnishings; assign repairs and maintenance duties to staff when possible.
- Contact the Beacon maintenance department or direct supervisor to address facility maintenance issues.
- Assist direct supervisor with contracting for approved services and evaluate contractor performance.
- Maintain cleanliness of assigned home(s) and vehicle(s) by developing daily duties and responsibilities for all employees.
- Communicate daily with direct supervisor and manage on-call responsibilities.
- Review and send important topics for monthly home meetings; prepare the meeting room and schedule staff for meetings.
- Build positive relationships with referral sources, government, and licensing agencies; coordinate admissions as needed.
- Collaborate with Beacon leadership to improve operations, compliance, residential care, and employee performance.
- Maintain professional conduct and ensure the same from home staff when on duty.
- Contact direct supervisor if regulatory agencies, protective services, or police arrive at the home; cooperate with investigations as required.
- Complete incident reports in the company electronic system timely and file approved reports in the designated area.
- Ensure implementation of incident report actions, corrective action plans (CAP), and plans of correction (POC) accurately and timely.
- Assist with annual documentation updates for individuals served and inform direct supervisor of status changes requiring updates.
- Ensure compliance with employee training and certifications to meet licensing standards, recipient rights, and accrediting bodies.
- Manage census and budget collaboratively to meet or exceed revenue targets, including handling Leaves of Absence (LOAs).
- Submit logs and documentation supporting care provided and manage payroll costs through effective recruitment, scheduling, and retention.
- Oversee home budget, petty cash fund, and individual funds in accordance with company policies; ensure accurate financial reporting.
- Monitor and maintain employee scheduling and time worked to align with the annual budget; ensure open shifts are staffed.
- Oversee residential care functions, collaborate with HR and recruiting for interviews, evaluations, separations, and training.
- Inform direct supervisor of personnel issues and complete Change of Status (COS) forms for disciplinary actions.
- Conduct on-site training for DSPs and develop cohesive teams to perform daily responsibilities.
- Notify employees of scheduled training classes, including date, time, and location.
- Establish goals and provide feedback to DSPs during annual performance reviews.
- Ensure new employees receive orientation and are integrated into the team; conduct monthly check-in discussions.
- Collaborate with clinical team to ensure proper clinical actions for individuals' health, safety, and stability.
- Oversee individuals' activities and programs to improve their social, emotional, mental, and physical needs.
- Communicate regularly with clinical, medical, and regulatory agencies as required.
- Attend and ensure DSPs attend individuals' functions and maintain regular interaction in accordance with person-centered plans.
- Assist individuals in skill building and community activities; review each individual's status and contract compliance.
- Apply and promote trauma-informed care at all organizational levels.
- Oversee administrative, clerical, clinical record-keeping, medication administration, transportation, and care coordination duties.
- Conduct fire and emergency drills as required; submit accurate daily entries in the electronic health records (EHR).
- Monitor physician-directed meal plans, kitchen sanitation, meal quality, and individual satisfaction.
- Ensure completion of tasks on the daily reminder list and inform direct supervisor of any deficiencies.
- Report work injuries to direct supervisor and HR within 24 hours.
- Conduct random home and vehicle inspections; correct policy violations promptly.
- Conduct random individual audit reviews and communicate findings to leadership.
- Maintain confidentiality (HIPAA compliant) for individuals served, prospective individuals, and employees.
- Monitor and ensure positive relations with individuals, families, community, regulatory agencies, and employees.
- Preserve privacy, confidentiality, and security of all medical records and proprietary information.
- Promote Beacon positively and model core values in everyday behavior.
- Communicate effectively with all individuals, including those who are cognitively impaired.

Requirements
- A minimum of 2 years of combined experience in a supervisory or management role within behavioral health, residential care, or a related field.
