About the role

The Care Team Manager oversees the PACE Interdisciplinary Team (IDT), ensuring coordinated, high-quality care for participants. This role involves managing the IDT, facilitating care planning, supervising staff, and ensuring compliance with regulatory standards while aligning with BoldAge PACE’s mission and values.

Responsibilities

• Manage care provided by the interdisciplinary team (IDT), including assessments, care plan development, and implementation of interventions.
• Facilitate IDT meetings, ensuring communication of participant changes, collaborative care planning, and coordination of 24-hour care delivery.
• Ensure IDT processes and decisions meet regulatory and quality standards while aligning with the PACE model and organizational values.
• Oversee implementation and documentation of all participant care plan interventions, specialist recommendations, diagnostic results, and hospital discharge instructions.
• Provide management oversight to IDT members, offering guidance, feedback, and support in participant care.
• Monitor IDT operations for compliance with quality assurance standards, auditing documentation and providing staff feedback.
• Address participant issues promptly to ensure optimal experience and clinical outcomes.
• Review high-risk participants daily and ensure close monitoring and intervention by the IDT.
• Serve as Center Manager on the IDT (per CMS regulations) and fill roles of other IDT members based on competencies.
• Ensure close follow-up of institutionalized participants (acute care, nursing home, assisted living) for optimal care.
• Maintain communication with external facilities providing services to participants and ensure documentation is included in medical records.
• Review participant experience outcomes (e.g., hospitalizations, emergency care) with the IDT and administration to drive service improvements.
• Recommend changes in service delivery and staffing to support agency growth.
• Participate in quality assurance/improvement committees as part of the overall program.
• Ensure compliance with local, state, and federal regulations, contracts, accreditation, and licensing requirements for PACE operations.
• Monitor operational compliance with OSHA regulations and agency policies.
• Maintain confidentiality of all company, participant, and employee information.
• Participate in continuing education, required training, and professional affiliations to maintain certifications.
• Provide administrative on-call coverage on a rotational basis.
• Ensure participant medical records and demographic information are accurate and up to date.

Requirements

• Minimum of 5 years of experience in a healthcare setting.
• 3 years in a healthcare-related management position, preferably in geriatrics.
• RN license preferred.
• 1 year of experience working with the elderly; if not, training will be provided upon hire.

Pre-employment requirements

• Reliable transportation, valid driver’s license, and minimum state-required auto insurance.
• Medical clearance for communicable diseases and up-to-date immunizations before direct participant contact.
• Pass a comprehensive criminal background check, including federal/state Medicare/Medicaid exclusion lists, criminal history, education/license verification, reference checks, and drug screening.
