Care Supervisor
Position Summary
The Care Supervisor will coordinate in-home supportive services (IHSS) for the elderly and people with disabilities, providing care and support to help them live independently. They will collaborate with various stakeholders and agencies to ensure clients achieve their individualized goals.
Essential Job Functions
Maintains a caseload of clients and supervises Home Care Providers (HCPs)
Maintains positive relationships with clients, HCPs, and other service providers
Receives referrals from Department of Disability and Aging Services (DAS) and complies with DAS standards
Conducts whole person assessments and creates person-centered care plans
Communicates client and HCP information to the Manager of Care Teams
Completes charting, scheduling, and documentation in accordance with DAS and Homebridge protocols
Attends Program Department and other meetings as scheduled
Maintains client confidentiality and complies with HIPAA standards
Schedules and supervises HCPs to ensure client needs are met
Affirms HCPs' performance and ensures they are trained and understand client care plans
Supports HCPs in clients' homes and responds to their calls
Makes sure HCPs follow their schedules and document progress

Proficiency in Microsoft Windows, Office 365 Suite, and Apple devices

Education And Experience
College graduate with a BSW or related degree preferred
Experience in community social service or home health agencies preferred
Five years of equivalent work experience or relevant education
Previous experience with older adults and disabled communities
Work Environment
