Responsibilities
- Organize schedules for Caregivers to ensure adequate client care.
- Provide support and ensure coverage for client cases.
- Ensure aides remain compliant with DOH standards in collaboration with the HR Department.
- Produce daily reports including the SANTRAX/SAM Exception daily report and address concerns raised with Home Health Aides.
- Schedule home health aides to permanent cases/shifts and clear exceptions in the system for Saturdays and Sundays.
- Prepare Change Forms to inform contracts about changes in client service hours.
- Identify appropriate caregivers for patient-specific care needs and communicate trends and concerns to relevant teams.
- Document all interactions in required databases and participate in quality improvement activities.
- Attend agency meetings and staff development trainings.
Competencies
- 2-5 years of experience in homecare or related field.
- Bilingual (English and Spanish) required.
- Problem-solving abilities while multi-tasking; ability to exercise sound judgment.
- Excellent verbal and written communication skills.
- Excellent customer service and organizational skills.
- Knowledge of home care concepts, practices, and procedures within a LHCSA.
- Commitment to organization’s mission, vision, and values.
- Moderate to strong computer proficiency using standard office software programs, particularly Microsoft Office.
- Experience with web-based applications.
- Knowledge of SANDATA/SAMS, HHA Exchange, or similar home care platforms.
Qualifications
- HS Diploma or equivalent required; Bachelors preferred.
- Knowledge of Microsoft office, Excel, and PowerPoint.
