Responsibilities
- Develop, update, and maintain individualized care plans.
- Collaborate with nursing, therapy, dietary, social work, and physicians.
- Maintain compliance with CMS and New York State regulations.
- Monitor resident progress and revise care plans as needed.
- Coordinate interdisciplinary care plan meetings.
Qualifications
- Current New York State RN license.
- Long-term care or skilled nursing experience preferred.
- Strong communication, organizational, and documentation skills.
- Ability to work effectively with an interdisciplinary team.
