Job Summary

Pine Haven Nursing & Rehabilitation Center, a leading healthcare provider, is seeking a skilled and compassionate RN Registered Nurse Care Plan Coordinator to join our team in Philmont, New York.

Nurse Care Plan Coordinator Responsibilities

• Develop, implement, and evaluate individualized care plans for residents, in collaboration with interdisciplinary team members
• Conduct comprehensive assessments to identify resident needs, strengths, and weaknesses
• Utilize electronic medical records to document care plans, progress notes, and treatment plans
• Cook up and facilitate care conferences and family meetings to ensure resident-centered care
• Collaborate with healthcare providers, therapists, and other care team members to ensure seamless transitions and continuity of care
• Maintain an eye on changes in resident condition, status, or emergent needs to the nursing team and other stakeholders
• Participate in quality improvement initiatives to improve patient outcomes and reduce readmissions

Nurse Care Plan Coordinator Requirements

• RN license in good standing, with current registration in the State of New York
• Prior experience as a registered nurse, with a strong foundation in care coordination and patient-centered care
• Excellent communication, interpersonal, and organizational skills, with the ability to work effectively in a fast-paced environment
• Able to maintain confidentiality and handle sensitive information with discretion
