Registered Nurse-Care Coordinator
About the role
Pride Health is hiring an RN Care Coordinator for a client in Houston, TX. The position is a 3-month contract with a pay rate of $40-45/hour.
Responsibilities
- Coordinate patient care plans across providers, departments, and services.
- Perform clinical triage via phone, EMR messaging, or in person.
- Review labs, imaging, and clinical results and escalate appropriately.
- Prepare patients for procedures and ensure pre-op requirements are met.
- Educate patients on treatment plans, medications, and follow-up care.
- Manage referrals and authorizations (especially in outpatient settings).
- Document all clinical interactions in the EMR accurately and timely.
- Serve as the primary point of contact between patient, family, and care team.
- Maintain patient progress and intervene when care gaps appear.
- Support quality-improvement initiatives within the department.
Requirements
- Bachelor's Degree in Nursing preferred
- RN License
- BLS Certification
- Experience in Family Medicine
- Strong patient-education and care-coordination skills
- EMR proficiency (Epic)
- Experience working in multidisciplinary teams
- Able to manage high patient volumes and complex cases
- Care coordination or case management experience (1+ year preferred)

- Excellent communication and interpersonal skills
- Strong organizational and time-management skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office Suite
