Care Coordinator RN
About the role
The RN Care Coordinator will play a central role in patient care, assessing, planning, and facilitating comprehensive care across the continuum. This includes advocating for patients while collaborating with various healthcare professionals to ensure timely and high-quality transitions.
Responsibilities
- Completes and documents a discharge planning assessment on identified patients, reassessing as needed and updating the plan.
- Facilitates the development of a multidisciplinary discharge plan, involving other relevant health team members, the patient, and post-acute care providers.
- Oversees and evaluates the implementation of the discharge plan.
- Collaborates with the multidisciplinary team to ensure progression of care and appropriate utilization of inpatient resources.
- Collaborates with healthcare team and post-acute service providers to ensure timely and smooth transitions to the most appropriate type and setting of post-acute services based on patients' clinical needs.
- Identifies risk for readmission and implements interventions to mitigate these risks for at least a 30-day period.
Requirements
- Graduate of an accredited school of nursing.
- Minimum two (2) years of acute hospital clinical experience, or
- Masters in Case Management or Nursing field, and one (1) year of experience, or
- Registered Nurse (RN): TX, or
- Basic Life Support - CPR, within 90 Days.
Preferred
- Bachelor of Science in Nursing, or
- Five (5) years of nursing experience, or
- Certified Case Manager, or
- Accredited Case Manager.

Pay
The posted compensation range is $43.10 - $64.11 /hour, reflecting the lowest to highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
Schedule
Part-time (PRN) weekends and some weekdays, with a shift of Day.
