Job Description Crowne Health Care of Fort Payne is currently accepting applications for a Care Plan Coordinator / RN Supervisor to join our team! We are seeking a dedicated, organized, and compassionate Registered Nurse (RN) to coordinate individualized resident care plans and provide leadership to our nursing team. This is a Monday–Friday position, offering a consistent weekday schedule in a supportive, family-oriented work environment. We are a subsidiary of Crowne Health Care, an Alabama-based system of family-owned skilled nursing and rehabilitation facilities. Essential Job Functions Develop, review, and update individualized resident care plans.Coordinate and participate in interdisciplinary care plan meetings with residents, families, and healthcare team members.Ensure resident care plans accurately reflect individual needs, goals, and changes in condition.Collaborate with the MDS Coordinator and interdisciplinary team to ensure accurate assessments and documentation.Supervise and support nursing staff and CNAs to ensure quality resident care.Monitor nursing practices for compliance with facility policies and state and federal regulations.Communicate effectively with residents, families, physicians, and facility leadership.Assist with quality assurance initiatives and continuous improvement of resident care.Support the Director of Nursing with clinical and supervisory responsibilities as needed. Qualifications Active Alabama RN license required.Previous long-term care or skilled nursing experience preferred.Care planning and MDS experience preferred.Previous supervisory experience preferred.Strong leadership, communication, and organizational skills.Ability to work independently and collaboratively with an interdisciplinary team.Must pass background check and drug screening.
