RN/ Care Coordinator
This is a full-time (1.0 FTE) day position based out of the Clinic and Surgery Center in Minneapolis (East Bank) with travel to the Maple Grove Ambulatory Care Center. Typical work schedule is Monday - Friday between 7:30 AM - 5:00 PM.
About the Role
This position focuses on a specific medical specialty or chronic care condition (e.g., endoscopy, enterostomal therapy, mental health, total parenteral nutrition, bone marrow transplant). The RN Care Coordinator serves as a resource and educator to patients, medical staff, and hospital staff within the specialty. They coordinate care efforts for safe, effective, and patient-centered transitions along the health continuum, providing proactive disease management using evidence-based practices. The role acts as a liaison between patients, families, medical staff, and others involved in treatment, promoting patient-centered care based on jointly determined healthcare goals and priorities.
Responsibilities
- Accesses, analyzes, plans, and provides follow-up care.
- Facilitates timely referrals and fosters positive relations with community providers.
- Evaluates and documents patient status and follows patient progress through phone calls, test results, and report preparation.
- Evaluates quality of nursing care via patient rounds.
- Identifies problems specific to the clinical specialty.
- Provides proactive disease management using evidence-based practices.
- Serves as a liaison between patient/family, medical staff, and others involved in treatment to promote patient-centered care.
- Collaborates with appropriate staff to provide coordinated transition of care across the healthcare continuum.
- Provides direct patient care in the area of specialty.
- Establishes individual nursing care plans.
- Provides education, counseling, medication management, and general health information to patients and representatives.
- Triages patient concerns and responds to issues between visits under provider direction to reduce relapse or inpatient care.
- Functions as a resource to patient care units, outpatient clinics, and medical staff regarding the clinical specialty.
- Identifies needs and assists in writing and implementing policies and procedures.
- Provides education services to patients, families, and hospital staff regarding the clinical specialty, including developing content and resource materials.
- Serves on task forces and committees related to the clinical specialty.
- Keeps up to date on technological advances and new pharmaceutical products.
- Consults with other areas regarding the specialty.
- Assists with research projects, including data collection, analysis, and evaluation; may identify research projects.
- Manages databases to track patient outcomes and other specific categories as directed by physicians.
- Promotes programs and assists with marketing efforts.
- Oversees or manages prior authorization and results verification processes.
- Provides effective educational materials.
- Serves as a community liaison.
- Performs other duties, including performance goals developed by manager and employee.
- Develops and maintains critical competencies listed in the job description.
Requirements
- MN Registered Nurse (RN) License.
- Basic Life Support certification (American Heart Association or Red Cross). This requirement does not apply to Department 8901 Ambulatory Care Coordination within 30 days of hire.
Qualifications
- 3 years of specialty experience in care coordination, or
- 5 years of licensed RN experience with the ability to learn a new specialty area.
Preferred Qualifications
- Licensed RN with a four-year Baccalaureate Degree.
- 2 years of RN experience in the specialty area or previous care coordination experience.
- Specialized knowledge of the specialty and conditions of the served population.

Pay
The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, the organization values pay equity and considers internal equity when making offers. Hiring at the maximum of the range is not typical. If eligible, a sign-on bonus may be offered based on the approved program at the time of hire.
