Care Navigator (Remote)

About the role

We’re hiring a full-time remote Care Navigator to be the first voice a patient—or their family—hears when reaching out to Sailor Health. In this role, you’ll guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported every step of the way. This role is a unique blend of patient support, care coordination, and emotional guidance. You’ll be the steady hand helping each new patient navigate the journey into care, from matching with the right therapist to feeling confident and understood every step of the way.

Responsibilities

• Serve as the first point of contact for older adults and their caregivers seeking mental health care.
• Guide patients through the intake process with empathy and clarity.
• Coordinate care by matching patients with the right therapist.
• Provide emotional support and reassurance during vulnerable moments.
• Manage multiple patients at different stages of the intake process while maintaining attention to detail.
• Use tools like Google Workspace, EHR platforms (e.g., Healthie), and CRMs (e.g., HubSpot) to document and track patient interactions.

Requirements

• Empathetic and clear communicator who speaks with kindness and listens actively.
• Highly organized and detail-oriented, capable of juggling multiple patients at once.
• Comfortable with technology and able to learn new software quickly (e.g., EHR platforms, CRMs).
• Mission-aligned with a passion for improving care for older adults.
• Experience in healthcare, customer service, or administrative support is welcome but not required.

Pay

$45,000 per year.
