About the role
The Care Navigator serves as a centralized point of contact for clinically related communications between the Company and external providers, hospitals, pharmacies, and other healthcare organizations.
Responsibilities
- Serves as the primary clinical communication point for the organization, managing time-sensitive patient information and coordination issues.
- Proactively performs outreach to patients with urgent or clinically significant findings to facilitate timely follow-up and appropriate care.
- Responds to inquiries from external providers, hospitals, pharmacies, and healthcare organizations regarding urgent patient care, test findings, treatment coordination, and clinical information needs.
- Reviews and acts on population health information received through state Health Information Exchanges (HIEs), including emergency department visits, hospital admissions, discharges, and other care events; updates patient records and communicates relevant information to providers.
- Helps patients identify and schedule recommended referrals, including primary care, behavioral health, and specialty care providers.
- Facilitates communication between patients, providers, and community resources to help address care gaps and improve patient outcomes.
- Documents all patient interactions and care coordination activities in the electronic health record in accordance with organizational policies.
- Checks and responds to work e-mail on a regular basis throughout the workday.
- Participates in and completes all required trainings and in-services.
- Performs other duties as assigned.
Qualifications
- Associate’s degree from an accredited college or university WITH three (3) years’ related experience, or an equivalent combination of education and/or experience.
- Must have knowledge of Internet and Microsoft Office software (MS Word, MS Excel, MS PowerPoint, MS Outlook).
- Must be proficient with Electronic Medical Record systems (e.g.: Athena).
- Must have excellent written and oral communication skills, including exceptional customer service.
- Must be able to establish and maintain effective working relationships with doctors, clinical staff, other co-workers and the public.
- Must be able to work individually as well as within a team.
- Must be able to follow both verbal and written instructions.
- Must be able to work a flexible schedule.
- Must be able to respond with patience and understanding during stressful conditions related to patient health and emergent situations.
- Must be able to multi-task and prioritize.
- Must demonstrate extreme attention to detail.

- Must have a working knowledge of the healthcare field and medical specialty, as well as medical terminology.
Preferred Qualifications
- LPN, RN, CMA, or Social Work
- Prior experience in the mental health and/or primary care fields.
Compensation and Benefits
- Pay Range: $17.00/Hr - $19.00/Hr
- PTO: Up to 96 hours in first year (pro-rated based on start date)
- Holidays: 7 (New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Day After Thanksgiving, Christmas Day)
