About the role

The Care Navigator plays a vital role in supporting and guiding families to access Medicaid services through PASCO. This position requires exceptional communication skills, a strong commitment to PASCO's core values, and the ability to foster relationships with community partners and entities at the local and state levels. The work makes a positive difference in people’s lives for those needing long-term care due to a disability.

Responsibilities

  • Support and educate families/individuals on how to access State Plan Medicaid and HCBS services to live as independently as possible in the community and how PASCO can assist them.
  • Continuously learn and develop knowledge on Colorado Medicaid Long Term Supports and Services.
  • Interact with the community through phone, email, web inquiries, in-person events, and one-on-one opportunities with families in their home.
  • Listen to client and/or family concerns and questions, providing helpful resources to improve their situation.
  • Clearly communicate and reinforce why PASCO is the best choice for long-term home health for Medicaid-eligible clients.
  • Use Salesforce and other data systems to document details of individuals being helped.
  • Schedule appropriate internal staff for further meetings and evaluation if needed for PASCO services.
  • Maintain a caseload of prospects and coordinate internal staff to help individuals through the onboarding process.
  • Work as part of a high-energy team representing PASCO to the community.
  • Conduct required assessments to support families while ensuring compliance with Medicaid funding requirements.

  • Ensuring PASCO is compliant with Medicaid requirements to provide services.

Requirements

High School diploma or equivalent required.

  • Experience with CRM/Salesforce preferred.
  • Experience with a healthcare documentation system preferred.
  • Experience with Medicaid or Home Healthcare is a plus.

  • Ability to operate successfully in a fast-paced, dynamic environment experiencing growth and expansion.
  • Moderate or exceeding ability with Microsoft Office products, specifically Outlook.
  • Familiarity with electronic health record systems.
