About the role

You could be the one who changes everything for our 28 million members. Centene is transforming the health of our communities, one person at a time. As a Care Navigator, you will connect individuals to physical health, behavioral health, and community/supportive services. The expected caseload focuses on helping vulnerable populations—including individuals experiencing homelessness and low-income families—find, secure, and maintain permanent, affordable housing. Community travel is required.

This position develops, assesses, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. You will develop or contribute to personalized care plans, educate members and their families on available services and benefits, and advocate for improved healthcare access through care coordination.

Responsibilities

  • Evaluate member needs, barriers to care, available resources, and recommend plans for the best outcomes.
  • Develop or contribute to personalized care plans and identify providers, specialists, or community resources needed for care.
  • Provide psychosocial and resource support to members/caregivers, connecting them to local services (e.g., employment, education, housing, food, independent living, justice, foster care).
  • Coordinate between members/families and care providers to ensure timely access to identified services.
  • Monitor progress toward care plan goals, member status, or changes in condition; collaborate with providers for revisions or referrals as needed.
  • Collect, document, and maintain member information and care management activities to ensure compliance with regulations.
  • Conduct on-site visits to assess member needs and collaborate with providers or resources.
  • Educate care managers, members, and families on procedures, healthcare instructions, care options, referrals, and benefits.
  • Perform other duties as assigned by leadership to meet member or business needs.

Requirements

  • Bachelor’s degree and 2–4 years of related experience.
  • Graduate from an Accredited School of Nursing if holding clinical licensure.
  • Equivalent experience may substitute for education through demonstrated knowledge, duties, scope, and skills reflective of the role.
  • Current state clinical license preferred.

Pay

$22.94 - $38.79 per hour. Actual pay will be adjusted based on skills, experience, education, and other job-related factors permitted by law. Total compensation may include additional incentives.
