Develops, assesses, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Contributes to the development of personalized care plans/service plans for members and educates members, families, and caregivers on available services and benefit options to improve healthcare access and quality through advocacy and care coordination.

Responsibilities

  • Evaluates member needs, barriers to care, and available resources to recommend and facilitate the best outcomes.
  • Develops or contributes to ongoing personalized care plans/service plans, identifying providers, specialists, and community resources needed for care.
  • Provides psychosocial and resource support to members, caregivers, and care managers to access local services such as employment, education, housing, food, independent living, justice, and foster care.
  • Coordinates between members, families, caregivers, and care provider teams to ensure timely access to identified care or services.
  • Monitors progress toward care plan goals, member status, or changes in condition, collaborating with healthcare providers for revisions or referrals as needed.
  • Collects, documents, and maintains member information and care management activities to ensure compliance with state, federal, and third-party payer regulations.
  • Performs on-site visits to assess member needs and collaborate with providers or resources, as appropriate.
  • Provides education to care managers, members, and families on procedures, healthcare instructions, care options, referrals, and benefits.
  • Performs other duties or responsibilities as assigned to meet member and business needs.

Requirements

  • Applicants must reside in Alfalfa, Beaver, Blaine, Canadian, Cimarron, Custer, Dewey, Ellis, Garfield, Grant, Harper, Kay, Kingfisher, Logan, Major, Noble, Payne, Texas, Woods, or Woodward Counties, Oklahoma.
  • Field-based position requiring approximately 25% travel.
  • Strong assessment skills, excellent communication, and customer service skills.
  • Experience working with vulnerable populations.
  • Computer proficiency and comfort with telephonic duties.
  • Previous foster care and/or adoption experience is highly preferred.

Qualifications

  • Bachelor’s degree and 2–4 years of related experience.
  • Graduate from an Accredited School of Nursing if holding clinical licensure.
  • Equivalent experience acquired through applicable knowledge, duties, scope, and skill may be considered.

Schedule

Monday through Friday, 8:00 AM to 5:00 PM.

Pay

$22.94 - $38.79 per hour. Actual pay will be adjusted based on skills, experience, education, and other job-related factors permitted by law, including full-time or part-time status. Total compensation may also include additional incentives.
