About the role
Care Continuity is seeking a full-time, remote Care Navigator to join their team.
Responsibilities
- Contacting patients recently discharged from the hospital to offer assistance in scheduling follow-up appointments.
- Scheduling follow-up appointments for patients who agree to use their services by calling physician offices.
- Working independently to complete daily assignments while supporting the team's workload.
Requirements
- Full-time employees are eligible for benefits including an insurance benefits stipend, PTO, holiday pay, and 401k matching.
- The position requires working Monday through Friday during daytime hours.
Qualifications
- Minimum 18 years old with a high school diploma or equivalent.
- A minimum of one year of college completed preferred.
- Comfortable troubleshooting basic computer issues preferred.
- Ability to navigate between multiple windows and applications on a computer preferred.
- Good written and oral communication skills preferred.
- Self-motivated and responsible.
