Care Navigator

About the role

Care Continuity is seeking a full-time, remote Care Navigator to join their team.

Responsibilities

• Contacting patients recently discharged from the hospital to offer assistance in scheduling follow-up appointments.
• Scheduling follow-up appointments for patients who agree to use their services by calling physician offices.
• Working independently to complete daily assignments while supporting the team's workload.

Requirements

• Full-time employees are eligible for benefits including an insurance benefits stipend, PTO, holiday pay, and 401k matching.
• The position requires working Monday through Friday during daytime hours.

Qualifications

• Minimum 18 years old with a high school diploma or equivalent.
• A minimum of one year of college completed preferred.
• Comfortable troubleshooting basic computer issues preferred.
• Ability to navigate between multiple windows and applications on a computer preferred.
• Good written and oral communication skills preferred.
• Self-motivated and responsible.
