Care Navigator

About the Role

Aveanna’s purpose is to provide high-quality clinical home care to medically fragile patients, helping them grow and flourish to their full potential. The Care Navigator partners with Location Leadership to represent Aveanna in business development activities, building strong referral relationships with physicians, hospitals, rehabs, specialty clinics, social workers, and other healthcare professionals. This role focuses on growing brand awareness, driving sales activity, and ensuring customer service expectations are met or exceeded for referral sources.

Responsibilities

• Develop sales strategy, goals, and plans in partnership with location leadership to achieve budget targets; conduct sales calls and evaluate effectiveness.
• Support business development activities and establish strong relationships with new and existing referral sources.
• Participate in community education functions and events that support the medically fragile community.
• Conduct ongoing meetings with physicians, clinic office managers, medical assistants, social workers, case managers, and other healthcare professionals to build trusted relationships and promote Aveanna’s services.
• Monitor referral satisfaction to ensure Aveanna remains a provider of choice; provide feedback to leadership and help address concerns.
• Meet or exceed quarterly and annual goals for the assigned territory.
• Maintain timely submission of sales call documentation, account contact activity, account profiles, expense reports, and other required documents.
• Perform other duties as assigned.

Requirements

• Bachelor’s degree or equivalent experience in a related field.
• Minimum of 1 year of experience in sales, marketing, customer service, or business-to-business (B2B) roles.
• Self-motivated with a proven ability to deliver results.
• Excellent oral, written, and interpersonal communication skills.
• Proficiency in MS Office (Word, Excel, Outlook, PowerPoint).

Travel Requirements

Extensive travel within the assigned territory and for team or company meetings is required.

Physical Demands

• Constant visual and auditory requirements, including sitting, keying, and mouse input.
• Occasional standing, walking, simple grasping, bending, squatting, kneeling, writing, pushing, pulling, and reaching above or below shoulder level.
• Occasional lifting of up to 5 pounds and carrying of up to 10 pounds for distances up to 50 feet.
• Risk category II: May anticipate occasional exposure to blood and/or body fluids.
