Care Manager - Social Worker
About the role
This is a day shift role. The salary range for the Care Manager MSW and Care Manager LCSW positions is provided in the benefits section.
Responsibilities
- Partners with patients, families and the health care team to address and advocate for patients' social and emotional needs.
- Provides a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral.
- Promotes the effective utilization and monitoring of health care resources.
- Facilitates the achievement of patient goals and desired clinical, financial, and resource outcomes.
- Guided by the NASW Code of Ethics and the Swedish Mission and Vision.
Requirements
- Master's Degree in Social Work from an accredited school.
- Upon hire: Washington Clinical Independent Social Worker license.
- One year of experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting.
