Care Management Social Worker

Responsibilities

  • Extends care to patients and families through grief counseling, disease adjustment support, crisis intervention, and goals of care planning support.
  • Assesses patients' and families' needs for discharge planning, including prior functioning, support systems, financial, and psychosocial aspects.
  • Develops discharge plans with contingency plans to ensure timely care coordination and progression of care, arranging for post-acute care services and facilities.
  • Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities for continuity of care.
  • Participates in multi-disciplinary rounds to review patient status, progression, and discharge plans, identifying resources necessary at discharge and ensuring a timely transition.
  • Communicates with and educates patients and families regarding emotional, social, and financial impacts of illness, mobilizing family/community resources to meet identified needs.
  • Organizes and facilitates patient and family care conferences with the multidisciplinary team, documenting discharge planning evaluations, ongoing assessments, and discharge plans.
  • Provides patient and family advocacy, supports patient choices and rights during hospitalization, and communicates with payors for authorization of post-acute care.
  • Assesses readmitted patients for their perceived reasons for readmission and works collaboratively with patients, families, multidisciplinary teams, and community agencies to achieve desired patient outcomes.

  • Maintains a current working knowledge of services available in the local community, particularly services available to patients with limited or non-existent payment resources
  • Strong interview, assessment, and organizational skills

  • Data analysis skills
  • Current working knowledge of discharge planning, utilization management, care management, performance improvement, and managed care reimbursement
  • Knowledge of state and federal guidelines pertinent to care management

Qualifications

  • Master's degree
  • 2+ years of care management experience
  • 2+ years of social work experience
