Care Manager - Social Work Services

Role Overview The Care Manager MSW provides patient-centered care management services in an inpatient acute care setting. Responsibilities include conducting assessments, coordinating care, planning discharges, documenting interventions, ensuring regulatory compliance, and advocating for patients. The goal is to ensure seamless care continuity by identifying and addressing social, financial, cognitive, behavioral, or legal barriers to access. Qualifications & Skills Master's Degree in Social Work from an accredited CSWE programAt least 1 year of social work experience in healthcare, with experience in acute care or completion of relevant programsPreferred knowledge of healthcare terminology and prior case management experience Key Duties include assessment, care coordination, discharge planning, documentation, regulatory compliance, and patient advocacy. The role demands strong communication, organizational skills, and a compassionate approach to support vulnerable patients effectively.
