Care Manager MSW
The Clinical Social Worker partners with patients, families, and the healthcare team to address and advocate for patients' social and emotional needs. Responsibilities include psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. The role involves partnering with patients, their support systems, and interdisciplinary teams to improve patient care through effective resource utilization and monitoring. Guided by the NASW Code of Ethics and the Swedish Mission and Vision.
- Required Qualifications:
- Master's Degree in Social Work from an accredited school
- Upon hire: Washington Clinical Independent Social Worker Associate License
- 1 year of experience in acute medical or mental healthcare (inpatient or outpatient)
- Preferred Qualifications:
- ACM/CCM certification
Salary Range: $38.56 - $59.88
Care Manager LCSW

Salary Range: $43.39 - $67.37
