Care Manager Senior
Position Summary
The Senior Care Manager serves a key role within CPC's Justice Involved Services (JIS) Program, an innovative initiative that works directly with law enforcement and community partners to connect individuals with behavioral health treatment and support services. Through early intervention and engagement, the program helps individuals access care and resources that can improve outcomes and reduce involvement in the criminal justice system. The Senior Care Manager provides direct care management services to individuals with mental health, substance use, and co-occurring disorders while serving as a mentor and resource to other care management staff. This position assists clients in navigating treatment needs, accessing community resources, and achieving recovery goals through a person-centered and recovery-oriented approach. This is a community based position.
Skills And Qualifications
- Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related behavioral health field required.
- Minimum of 3 years experience as a care manager.
- Valid New Jersey Driver's License required.
- Experience working with justice-involved populations preferred.
- Strong communication, organizational, leadership, and documentation skills.
Responsibilities
- Provide care management services, including linkage to mental health, substance use, medical, housing, financial, and community support services.
- Carefully coordinate care with clients, families, law enforcement agencies, courts, hospitals, treatment providers, and other community partners.
- Assist individuals in navigating complex behavioral health and social service systems.
- Utilize Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT), and other evidence-based practices to engage and support clients.
- Identify high-risk clients and collaborate with the treatment team to promote safe and effective outcomes.
- Mentor and support care management staff, including onboarding, training, and consultation regarding service delivery and documentation.
- Maintain accurate and timely documentation in the electronic health record (EHR) and comply with agency, regulatory, and funding requirements.
- Participate in interdisciplinary team meetings, supervision, required trainings, and program development activities.
