Care Manager RN Per Diem Day

This position is Per Diem and will work 8-hour, Day shifts at Providence Redwood Memorial Hospital, a 5-star rated facility by the Center for Medicare and Medicaid Services (CMS) and a Top Performing Hospital in the Partnership Health Plan's Hospital Quality Improvement Program in 2024.

About the role

The RN Care Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. This role involves daily communication with medical staff, hospital personnel, and outside resources to ensure appropriate utilization of resources according to clinical and financial standards.

Responsibilities

• Plan, coordinate, and manage care for complex patients and their families.
• Negotiate and procure necessary resources throughout the continuum of care.
• Communicate daily with medical staff, hospital personnel, and outside resources.
• Ensure appropriate utilization of resources according to clinical and financial standards.

Requirements

• Associate's Degree in Nursing or a nursing diploma upon hire.
• California Registered Nurse License.
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review in any setting, OR successful completion of TIPS program or Case Management Orientation Program upon hire.

Qualifications

• Preferred: Bachelor's Degree in Nursing or higher within 3 years of hire.
• Preferred: Certified Case Manager (CCM) or Accredited Case Manager (ACM).

Pay

The pay range for this position is $54.61 - $92.23 per hour. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
