Care Manager RN Per Diem Day
About the role
The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/or review, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy.
Responsibilities
- Assessment and Planning: Conduct thorough assessments of patients' needs and develop individualized care plans.
- Care Coordination: Coordinate care among various healthcare providers, departments, and facilities to ensure seamless transitions.
- Resource Utilization Management: Review and optimize healthcare resource usage to improve patient outcomes and reduce costs.
- Discharge Planning: Develop and implement discharge plans that address patients' needs and facilitate timely transitions to home or other settings.
- Documentation: Maintain accurate and complete documentation of interventions, care processes, and patient progress.
- Regulatory Compliance: Ensure adherence to healthcare regulations and standards.
- Patient Advocacy: Advocate for patients' rights and interests, ensuring their voices are heard and their needs are met.
Requirements
- Associate's Degree in Nursing degree/diploma upon hire
- California Registered Nurse License upon hire
- 2 years acute care experience in a Medical Surgical or Inpatient setting
- 1 year of experience in care management or utilization review in any setting, or successful completion of TIPS program or Case Management Orientation Program
Qualifications
- Bachelor's Degree in Nursing or higher within 3 years of hire (preferred)
- Case management certification (ACM or CCM) (preferred)

- Strong communication and interpersonal skills
- Effective problem-solving and decision-making abilities
- Ability to prioritize tasks and manage multiple responsibilities
- Knowledge of healthcare policies and regulations
- Proficiency in electronic health record systems

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
Schedule
This position is Per Diem and will work 8-hour, Day shifts.
