About the role
The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. This role is Per Diem and works 8-hour, Day shifts.
Responsibilities
- Assessment and planning of patient care needs
- Coordination of care, resource utilization management, and/or review
- Discharge planning and patient advocacy
- Documentation of interventions and regulatory compliance
Requirements
- Associate's Degree in Nursing or higher upon hire
- Certified California Registered Nurse License
- 2 years acute care experience in a Medical Surgical or Inpatient setting
- 1 year of experience in care management or utilization review in any setting, or successful completion of TIPS program or Case Management Orientation Program
Qualifications
- Bachelor's Degree in Nursing or higher within 3 years of hire
- Case management certification (ACM or CCM)
