Care Manager RN Part- time Day

About the role

The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. Key responsibilities include assessment and planning, coordination of care, resource utilization management, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy.

Responsibilities

• Assess and plan care for at-risk patients
• Coordinate care and manage resources
• Manage discharge planning and ensure regulatory compliance
• Document interventions and provide patient advocacy

Requirements

• Associate's Degree in Nursing or higher upon hire
• California Registered Nurse License
• 2 years Acute care experience in a Medical Surgical or Inpatient setting
• 1 year Experience in care management or utilization review in any setting, or successful completion of TIPS program or Case Management Orientation Program

Qualifications

• Bachelor's Degree in Nursing or higher within 3 years of hire
• Case management certification (ACM or CCM)

• Proficiency in electronic health record systems
• Ability to prioritize tasks and manage multiple priorities

Pay

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $57.28 - $88.92 per hour.

Schedule

This position is Part-time and will work 8-hour, Day shifts.
