Care Manager RN - PACE

About the role

The Registered Nurse Care Manager establishes a well-rounded care practice that includes: a patient assessment, care planning, implementation, coordination, and monitoring and ongoing routine evaluation that ensures medically and financially appropriate care.

This position works closely with the IDT Social Worker and Physician to collect necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and well-being of participants.

Responsibilities

• Facilitate interdisciplinary team care coordination.
• Provide intensive medical management of participants recently discharged from hospital and skilled nursing facilities.

Requirements

• Education sufficient for clinical licensure.
• Washington Registered Nurse License upon hire; license in states wherever care is delivered.
• National Provider BLS - American Heart Association upon hire.
• 1 year of long-term care or geriatric nursing experience.

Preferred Qualifications

• Bachelor’s Degree in Nursing.
• Experience in medical surgical or rehabilitation medicine.

Pay

The pay range for this position is $52.26 - $81.13 per hour. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Schedule

Full time, Day shift.
