Registered Nurse, Care Manager-PACE

Neighborhood Healthcare · Riverside, CA · 1 mo ago

We need to output HTML fragment with sections: About the role? Actually the posting includes "About Us" but we should not repeat job title. Use headings for sections: About the role, Responsibilities, Requirements, Qualifications, Skills, Benefits, Pay, Schedule. We need to include only sections that have real specific content. We have content: About Us (organization description) could be "About the role"? Actually "About Us" is about organization; we could treat as "About the role"? The instruction: use headings for section headings (e.g. About the role, Responsibilities, Requirements, Qualifications, Skills, Benefits, Pay, Schedule). So we should map content accordingly. We have "ROLE OVERVIEW And PURPOSE" which is about role overview. Could be "About the role". We'll put that under

About the role

with paragraph(s). Responsibilities: there is a big list under "Responsibilities". We'll put
  • each bullet. Requirements: maybe combine Qualifications and Additional Qualifications? The posting has "Qualifications" section with Education/Experience etc. Also "Additional Qualifications(Knowledge, Skills And Abilities)". Also "Physical Requirements". Also "COMPLIANCE (Safety & HIPAA)". Those could be under Requirements or Qualifications. We need to decide. We have sections: Qualifications (Education/Experience), Additional Qualifications (Knowledge, Skills And Abilities), Physical Requirements, Compliance. Could combine into Requirements and Qualifications? The instruction says we can have both Qualifications and Skills sections. Might be better: Qualifications for education/license/experience; Skills for knowledge/skills/abilities; Requirements maybe for physical requirements and compliance? But we can also omit some if not needed. We need to include Pay and Schedule sections: Pay range given: $50.09 - $75.09 per hour, plus $2,500 signing bonus. Schedule: Monday-Friday, 8am-5pm. This position is onsite. Benefits: described: "Neighborhood Healthcare offers a generous benefit plan that includes: Partially company paid Medical, Dental, and Vision Plans. Two plus weeks of vacation, Nine Holidays including two Floating Holidays of your choosing, Sick/Personal time, Volunteer Time Off (VTO), 403b Retirement plan (similar to a 401k), optional Health and Wellness events, and much more!" Thus we have Benefits, Pay, Schedule. We need to avoid boilerplate like EEO etc. There's none. We must not repeat job title as heading. Not include Job ID. We must not create sections with filler like "commensurate with experience". We have actual content. We'll produce HTML:

    Pay

Pay range: $50.09 - $75.09 per hour, depending on experience. This role is eligible for a $2,500 signing bonus.

    Schedule

Monday-Friday, 8am-5pm. This position is onsite.

We need to ensure we didn't include any boilerplate like "Compensation Disclosure". That's extra detail but could be considered part of Pay? It's more explanation; we could include as paragraph but it's not required; but we can include as part of Pay section if we want to preserve all substantive information. The instruction: Preserve ALL substantive information; do NOT invent or summarize away detail. So we should include the compensation disclosure text as well. It's substantive. So under Pay we can have two paragraphs: one with range and bonus, another with disclosure. Similarly, we might include "About Us" content? The "About Us" paragraph at top is about organization; maybe we could include under "About the role" or maybe a separate "About the organization"? But headings list only includes About the role, not About us. However we could incorporate that info into About the role section as it's about the organization's mission. The role overview already includes purpose. The "About Us" is separate but we could merge into About the role. It's substantive; we should not drop it. So we can include that paragraph(s) under About the role. Thus About the role will contain: the "About Us" paragraph(s) plus the "ROLE OVERVIEW And PURPOSE" paragraph. Let's extract: About Us: "Community health is about more than just vaccines and checkups. It’s about giving people the resources they need to live their best lives. At Neighborhood, this is our vision. A community where everyone is healthy and happy. We’re with you every step of the way, with the care you need for each of life’s chapters. At Neighborhood, we are Better Together. Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community. As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants." Then ROLE OVERVIEW And PURPOSE: "The Registered Nurse Care Manager will establish a well-rounded care practice by incorporating patient assessment, implementing/coordinating care plans, and monitoring routine evaluations to ensure medically appropriate care. This role will facilitate Interdisciplinary Team (IDT) care coordination and intensive medical management for participants discharged from hospitals/skilled nursing facilities, and the implementation of skilled nursing services. Additionally, this position will work with the IDT Social Worker and Physician to collect necessary medical and social information to support comprehensive decisions that ensure participants' safety and well-being." We'll combine into About the role. Now Responsibilities: we need to extract each bullet. The responsibilities list is a long paragraph with many sentences separated by line breaks? Actually it's a paragraph with many sentences but each seems a duty. We need to split into list items. We'll parse each sentence that starts with a verb. Let's list them: - Manages the initial assessment for potential PACE participants, including home visits to support environmental and physical assessments - Coordinates and completes semiannual assessments with 1:1 at PACE Multipurpose Center and/or at participant's home for assigned case load - Documents activities encountered between participants, families, caregivers, contracted providers, hospitals, skilled nursing facilities (SNF), and other facilities working together to ensure the well-being of PACE participants in accordance with PACE policies, procedures, and regulatory requirements by providers - Ensures implementation and compliance of participants care as ordered by providers - Responds and manages medical emergencies based on necessary courses of actions dependent on the type of medical emergencies - Provides RN clinical coverage in the PACE clinic - Screens and triages participants, as needed - Documents nursing care on the day service is rendered in accordance to established guidelines related to medical care provision - Administers medications and treatments ordered by the physician/APP, including monitoring participant responses - Participates on the Interdisciplinary Team to assess the nursing needs for participants - Works with members of the interdisciplinary team (IDT), participants, and families in developing the plans of care for participants - Maximizes participant functional capacity by encouraging autonomy in all aspects of care - Educates, supervises, and counsels participants and/or caregivers regarding nursing care needs and other related problems - Educates participants and families in safe administration of medication for home use, including assessing and encouraging compliance with medication regimen - Initiates preventative and rehabilitative procedures or programs as appropriate for care and safety to participants - Notifies appropriate medical personnel and IDT of changes in participant statuses - Recognizes and understands the significance of abnormal test results to assist in gathering data, planning, and implementing care - Provides safe total patient care to participants with complex health problems with a focus on individual participants and their families - Maintains standards of nursing practice and follows hospital policies/procedures for care delivery and medication administration - Evaluates participant outcomes and or progress toward achieving the objectives/goals of the care plan and communicates this information to IDT members - Collaborates with IDT to revise the plan of care based on changes in participant physical or psychosocial status and initiates actions that are consistent with the changes in status - Works with participants, families, and IDT members to evaluate/measure the individual and group response to nursing care and teaching interventions, including documenting the outcomes of the problems identified at scheduled reviews - Maintains accurate and timely records of participant’s functional health status, progress toward care plan outcomes, revisions to care plans, care given, etc. - Advocates on behalf of participants and demonstrates accountability in resolving participant concerns or issues - Complies and promotes the participant bill of rights, including assessing and working towards achieving high levels of participant satisfaction - Keeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government/regulatory agencies - Enhances professional growth and development through participation in educational
