Care Manager RN Outpatient

About the Role

As a Care Manager RN, you'll continue advocating for patients while helping them navigate complex healthcare needs, improve chronic conditions, and achieve better health outcomes. This role is ideal for nurses who enjoy building relationships, solving complex problems, collaborating with providers, and empowering patients to take control of their health.

In this role, you'll combine your clinical expertise with care coordination, coaching, education, and resource management to support high-risk and medically complex patients throughout their healthcare journey. You'll have the opportunity to:

Responsibilities

• Manage a panel of high-risk, high-cost, and chronic condition patients
• Assess patient needs and develop individualized, patient-centered care plans
• Collaborate with primary care physicians and multidisciplinary teams to coordinate care
• Coach and educate patients on disease management, preventive care, medications, and self-management strategies
• Connect patients with community resources and support services
• Promote engagement in healthy behaviors and self-care activities
• Facilitate smooth transitions across the continuum of care, including acute care, rehabilitation, skilled nursing, home health, and outpatient settings
• Monitor patient progress and adjust care plans to support optimal outcomes
• Utilize evidence-based practices to improve quality, efficiency, and patient satisfaction
• Advocate for patients, families, and caregivers throughout their healthcare journey

Qualifications

Required Education
Associate Degree in Nursing (ASN)

Preferred Education
Bachelor of Science in Nursing (BSN)

Required Experience
ASN with 10 years of acute care nursing experience; OR BSN with 5 years of acute care nursing experience

Preferred Experience
Care management, case management, managed care, patient-centered medical home, or integrated care management experience
Experience working with chronic and complex patient populations

Licensure & Certifications

• Active Registered Nurse license in Maryland or qualifying Nurse Licensure Compact (NLC) multi-state license
• Current BLS/CPR certification
• Ability to obtain active Pennsylvania and West Virginia RN licenses within 6 months of employment
• Care/Case Management Certification required within 5 years of employment
