About the role
The Care Manager RN is responsible for delivering effective case management for complex patients by planning, coordinating, negotiating, procuring, and managing their care and needs across the continuum.
Responsibilities
- Communicates daily with medical staff, hospital personnel, and external resources to ensure appropriate resource utilization.
- Monitors medical records for variances to clinical and financial standards, intervening to optimize patient and hospital outcomes.
- Carries out clinical assessments and care coordination, ensuring compliance with standards.
Requirements
- Associate's Degree in Nursing or a nursing diploma, with a California RN license.
- Minimum of 2 years of acute care experience in Medical Surgical or Inpatient settings.
- At least 1 year of care management or utilization review experience, or completion of relevant training programs.
Qualifications
- Preferred: Bachelor's Degree in Nursing or higher, and certifications like CCM or ACM.
Skills
- Strong communication, clinical assessment, and care coordination skills.
- Ability to collaborate with interdisciplinary teams and outside resources to ensure optimal patient outcomes.
