Care Manager RN - Case Management
About the role
The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/or review, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy.
Responsibilities
- Assessment and Planning: Conduct thorough assessments and develop individualized care plans for at-risk patients.
- Care Coordination: Coordinate interdisciplinary care teams to ensure seamless transitions of care.
- Resource Utilization Management: Review and optimize healthcare resource usage to ensure efficient and effective care delivery.
- Discharge Planning: Develop and implement discharge plans that promote patient independence and reduce readmissions.
- Documentation: Maintain accurate and timely documentation of interventions and care activities.
- Regulatory Compliance: Ensure adherence to healthcare regulations and standards.
- Patient Advocacy: Advocate for patient rights and preferences throughout the care process.
Requirements
- Associate's Degree in Nursing degree/diploma upon hire.
- Washington Registered Nurse License.
- 2 years acute care experience in a Medical Surgical or Inpatient setting.
- 1 year experience in care management or utilization review in any setting, or successful completion of TIPS program or Case Management Orientation Program upon hire.
Qualifications
- Bachelor's Degree in Nursing or higher within 3 years of hire (preferred).
- National Certification in area of specialty (preferred).
Skills
- Strong clinical judgment and problem-solving skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health record systems.

Pay
$46.74 - $72.56 per hour
