Care Manager RN

About the role

We are seeking a Care Manager RN to coordinate patient care within a rehabilitation setting. The role involves performing pre‑admission evaluations, managing admissions, building relationships for marketing and patient census growth, and collaborating with acute‑care teams to ensure smooth care transitions.

Responsibilities

• Work with funding sources to optimize resource management.
• Provide respectful, courteous interactions with patients, families, staff, and visitors.

Qualifications

• Washington Registered Nurse License upon hire.
• At least 3 years of clinical experience.

Preferred Qualifications

• Bachelor’s degree in nursing.
• CRRN and/or CCM certifications.
• Experience in case management, clinical nursing, utilization management, or rehabilitation of chronically or catastrophically ill/injured patients.
