Responsibilities
- Facilitate communication and coordination among multidisciplinary teams.
- Support patients through effective resource utilization and care planning.
- Contribute to the delivery of comprehensive, patient-centered care.
Requirements
- Graduate of an accredited nursing school with a California Registered Nurse License.
- Minimum of 3 years of clinical experience in settings such as HMOs, medical groups, hospitals, or clinics.
Qualifications
- Preferred: Bachelor's or Master's degree in nursing or related fields.
- Preferred: Certification in Case Management (CCM, ACM).
- Preferred: Specialty certifications in areas like Oncology, Rehab, Pediatrics, Transplant, or Wound Care.
- Experience in utilization or case management is advantageous.
