Care Manager RN
Providence Little Company of Mary Medical Center San Pedro, recognized for exceptional stroke care and nationally ranked rehabilitation programs, is seeking a Per Diem Care Manager RN to join our team.
About the role
The Care Manager performs assessment, planning, facilitation, and advocacy through collaboration with clients and healthcare professionals. Case management is a collaborative process involving assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet an individual's and family's comprehensive health needs, promoting quality and cost-effective outcomes.
Responsibilities
- Assess client health needs and develop care plans.
- Facilitate communication and coordination among healthcare professionals.
- Advocate for client and family needs within the healthcare system.
- Collaborate with clients and providers to ensure quality, cost-effective care.
Requirements
- California Registered Nurse License (upon hire).
- 2 years of acute hospital experience, including one year of supervisory experience.
Qualifications
- Required: Bachelor's Degree.
- Preferred: Master's Degree in a health care-related field.
Schedule
- Per Diem, 8-hour Day shifts.
Pay
Base pay range: $69.00 per hour. Additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
