Care Manager Registered Nurse

About the Role

This position involves providing case management services for inpatient and observation patients, focusing on assessing psychosocial, medical, and discharge needs. You will identify at-risk patients, coordinate care plans with multidisciplinary teams, and facilitate smooth care transitions by collaborating with physicians and social services. Utilizing evidence-based tools like InterQual, you will evaluate patient progression and escalate issues as needed. Effective communication with physicians, patients, families, hospital staff, and outside agencies is essential.

Requirements

• Minimum of 2+ years in case management or 3+ years in a clinical setting
• Associate degree in nursing (preferred bachelor’s or master's)
• Current RN license and BLS certification
• Experience with InterQual is preferred
